The modern society in the United States is never comparable to the past. There are a lot of changes which have been witnessed both in the culture, policy, and politics. Prior to the civil war era and immediately after the civil war era. There as a different perception of race and culture. This, therefore, led to establishing the ethnic neighborhood, churches schools, and other amenities. The Americans who lived in the populated areas which were mainly the urban areas had the designation of neighborhood based on the races.   In addition to the perception which the Americans had in regard to some races by Americans, the political leadership, the policymakers and the society at large never condemned the racial segregation which was clearly embraced through not only the day to day interaction but also the way of life which was characterized by the neighborhood arrangements.

As recorded in the Library of Congress in the article entitled Mark Twain’s  Observation About Chinese Immigrants in California, It is clear that the modern urban is run under a fair law that seeks to bring equality among people of all races (The Library of Congress). The article reports a case of Chinese who was stoned to death by some boys without intervention by onlookers. Modern society and the law as do not allow such illegal actions and thus clearly showing that equality and the rule of law are advocated for by the law enforcers which has seen most people being shown why equality is fundamental (Booker T Washington speech).

In conclusion, the United States if not the World has undergone civilization which has seen people of different races acquiring education and participating in building the nation regardless of their races or culture (Jane Adams). This has therefore brought people from different parts of the country live together in urban areas in harmony, a thing that never existed in the 1880s and before.  This has therefore brought peace prosperity in the urban areas as well as in the rural areas.

Contemporary Social Justice Issue 

The US opioid overdose crisis is a social justice and public health emergency. The death rate of drug overdose more than tripled from 1999 to 2017. The public stigma surrounding opioid use continues to fundamentally hinder the response to the opioid overdose crisis, causing people with opioid use disorders to disengage from care, resulting in poorer health outcomes, psychological distress, and poorer quality of life (Tsai, et al., 2019).  The opioid overdose crisis is a complex, multifaceted, public health emergency requiring evidence-based primary, secondary, and tertiary preventive interventions.

One Related Historical Social Justice Issue 

With the Covid-19 pandemic, women have been disproportionally affected by the opioid epidemic and the physical and biosocial effects of opioid use specific to women are an important consideration for healthcare providers. Cook (2022) reports recent data show that the effects of the opioid epidemic on rates of opioid use disorder (OUD), overdoses, and the economy continue to rise, despite global efforts to understand the drivers and develop effective prevention and intervention strategies, programs, and policies. 

What Stands Out and Manifestation 

An opioid epidemic has been happening worldwide since the 1990s and continues to impact individuals, families, communities and societies around the globe. Cook (2022) reports that women have been disproportionally affected by the opioid epidemic and the physical and biosocial effects of opioid use specific to women are an important consideration for healthcare providers. The epidemic has evolved from heroin misuse to the use of synthetic opioids that are easily manufactured and are readily available. Reasons for the continuing opioid epidemic are complex, and include factors related to mental health, addiction, and chronic pain relief. Pregnant women aren’t immune to the opioid epidemic ravaging the country, and these babies are more likely to be smaller, and as many as 80% of the babies have withdrawal symptoms.

Contemporary Social Justice Issue 

The contemporary social justice issue chosen was gun violence.  Gun violence affects people of ALL ages and races in the United States, but it disproportionately impacts young adults, males, and racial/ethnic minorities (Beardslee, Docherty, Mulvey, & Pardini, 2021).  Just recently, in a small county in Mississippi (near my HOME), a man, age 25 years old, allegedly murdered two innocent children ages 1yr and 9 years old at gunpoint on January 9th.  Gun violence is one of the leading causes of premature deaths in the U.S.  Each year, approximately 117,345 people are shot (Brady Campaign, 2023).  According to the Brady Campaign, approximately 40,620 people die from gun violence, 15,343 are murdered, 76,725 people survive gunshot injuries and 34,566 are intentionally shot by someone else and survived.  The ongoing gun violence is very disconcerting  and one of the solutions to address gun violence is to include a national system of licensing and registration along with comprehensive background checks.

One Related Historical Social Justice Issue 

The Charleston church shooting was one of the most disheartening and devastating events that has transpired in the United States.  The shooting happened inside the Mother Emanuel AME, which is an African Methodist Episcopal, Church in the city of Charleston on June 17, 2015.  The white supremacist, Dylan Roof, assassinated 9 African Americans, in the middle of bible study and fled the scene without injury (Della Cava, 2020).  When the news of the shooting was learned, I was furious, angry, emotional, distraught to hear that 9 innocent people was murdered inside a church, where it is considered for some of us as a “safe” place. 

What Stands Out and Manifestation 

The issue that stands out the most is the easy access to gun and the lack of action that the United States is taking to restrict access to guns in all states.  Compared to other countries, the United States have the highest percentage of gun violence.  This can be due to the decease of community safety, unsupervised access to guns, and involvement in risky community behaviors such as drug dealing and usage (Beardslee, Docherty, Mulvey, & Pardini, 2021).

Sudden cardiac arrest in the medical world is termed as a condition whereby the heart unpredictably stops beating. The act of the heart stopping to beat interferes with the blood flow to the brain and other vital organs in the body. The stopping of blood flow in the body is attributed to an electrical disturbance in the heart (Miller, 2013). If SCA is not treated immediately one might die. Sudden cardiac arrest is distinct from heart attack; the latter occurs once blood flow to a portion of the heart is stopped. Sudden cardiac arrest is considered a medical emergency since delayed treatment may result in death. The kind of medical care that can be administered to an individual in such circumstance is cardiopulmonary resuscitation. Another form of medical care that can be employed is the use of a defibrillator. The defibrillator is a medical tool which sends electrical currents to the heart in a bid to restore the normal heartbeat. The public can use an automated external defibrillator to revive an individual who has just suffered a sudden cardiac arrest (Hidenreich et al., 2013).

Policy Priority Issue

 Sudden cardiac arrest has affected society by threatening the health of different population segments. As stated above, cardiovascular diseases are more often than not related with the elderly. Sudden cardiac arrest has proved more devastating than other diseases because it happens to the younger generation. It is even more unfortunate because it often happens to young adults and adolescents ranging from 0.5 up to 20 for every 100,000 individual years (Berger, 2012). It should be tallied among the most dangerous diseases and placed as one of the major priorities for disease control organizations and institutions. Sudden cardiac arrest has become a serious health problem for adolescents since less than 10% of students affected survive these attacks. The Centers for Disease Control and Prevention in the United States estimate about 2200 people under the age of 26 dying of SCA. The paper will provide an insight to the policy analysis about sudden cardiac arrest. It will also highlight on the feedback of a policymaker concerning sudden cardiac arrest.   

Policy Issue Analysis

There have been policies aimed at reducing the effects of sudden cardiac arrest in schools. One of them is pre-participation physical evaluation. This is the requirement that all student athletes be tested for any cardiovascular disease before participating in any field activity. This is aimed at eliminating the students who are bound to be negatively affected by highly strenuous exercise routines. However, controversy on the issue is based on the fact that the effectiveness of simple fitness tests is limited against the diverse causes that may cause Sudden Cardiac Arrest. Furthermore, surveys have shown that though this system should be implemented in all schools, the rules accompanying the exercise are a little or not at all understood by most school authority bodies. The compliance levels for the guidelines are therefore minimal which demeans the purpose for which the policy was developed. This requires amending policies or other supporting policies to help bridge the gap of implementation in schools (JC, 2013).

Currently, many business enterprises do not have AEDs since they have the notion that SCA will not happen in such a place. A number of businesses overlook this device since they argue it incurs necessary expenses on businesses. However, California has imposed a law (the CA Bill 1507) requiring all health clubs to have an AED. The state of California implements the ‘Good Samaritan Law’ to safeguard the operator of an AED as long as he/she uses the given guiding principles. However, increasing AEDs did not translate to improved survival rates. Therefore a proper way of dealing with this concern would be to introduce SCA screening.

Recommendations

Because the aforementioned policies were not effective as they were meant to be, a number of recommendations have been lined up to provide a long term resolution. Critics of the pre-participation physical evaluation are of the opinion that the practice should be broadened to encompass students who are not athletes. Alternatively, critics of the CA Bill 1507 which proposed the use of AEDs in public places argued that the adoption of such policies did not increase the mortality rate of individuals. Therefore, they opted for implementing SCA screening because it would lead to fewer deaths contributed to SCA.

How can you find out the current status of this issue?

The issue’s current status (SCA) can be obtained from the National EMS Information System (NEMSIS). NEMSIS strives for the consistent collection of EMS data. The driving forces behind NEMSIS are the Centers for Disease Control and Prevention, the National Highway Traffic Safety Administration and finally the Health Resources and Services Administration. Thus far, all states have adopted the NEMSIS which contains all patient records regardless of whether you are treated for SCA or any other disease. The benefits that accrue from the NEMSIS program are the standardized collection method. Nevertheless, the data collected by the National EMS Information System may fail to portray all the EMS activities within the state. The reason behind the aforementioned statement is that the data collected through this method is self-reported and misses important status thus its maintenance is restricted.

The Level of Government That I Target

The level of government that I target is the Federal government. The federal government has members of congress who are called representatives. This arm of the government is charged with the responsibility of making laws by approving bills. This fact leads me to Jerry L. Stebelton who is the state representative serving in the Ohio house of representative. To further research on the policies passed or in progress concerning the issue, I scheduled a meeting with Jerry L. Stebelton, the state representative serving in the Ohio house of representative for his fourth term. He represents the 77th District. Jerry Stebelton has influence on the education sector because he is the chairman of the education committee in the Ohio House of Representatives. Through this detailed discussion I discovered that Representative Stebelton is further driven towards the elimination of sudden Cardiac Arrest in schools because his son was diagnosed with Cardiac Arrhythmia at an early age. Having this position and insight about the disease, his efforts are highly advantageous in the policy making process concerning the Sudden Cardiac Arrest Issue.

His main point of consideration is that other than concentrating on testing and preventing students with heart conditions from participating in sporting activities, there should be preventive measures in place to help those who get these attacks even without participating in sports. This would include well trained personnel who would be the first responders to such attacks. The earlier the condition is recognized and treated, the higher the chances of survival will be. Jerry Stebelton is therefore looking into the development and implementation of policies that will help the treatment of students with such attacks right at the school therefore reducing the chances of sudden cardiac death (SCD) while on their way to hospital or awaiting treatment. This approach was encouraged by the fact that in the recent past two students in the local school system have experienced sudden cardiac arrest but survived as a result of timely response by bystanders and EMS. This emphasizes that measures to encourage responsiveness may save more lives in the future.

Representative Stebelton has used some of the existing house bills to try and reduce the increasing effects of sudden cardiac arrest in adolescents. He has done this through proposing some alterations in the bills and also actively supporting the positive aspects of the existing bills to further his cause. His actions are well illustrated through his actions concerning the House Bill no. 247.  The House bill no. 247 states that those who operate an AED must be CPR certified. In support of this, Jerry Stebelton has continued to fight that all teachers become CPR certified in order to increase the number of personnel eligible to respond to cases of sudden cardiac arrest in schools. On the other hand, he proposed an amendment to House bill no. 247 to exclude the clause that requires all responders to be CPR certified. This would create the opportunity for bystanders and other untrained personnel to save lives without the inhibition of certification (General Assembly of the State of Ohio, 2013).

However, if this bill passes, there will be a need to train these teachers on conducting first aid and using an AED. This would prevent the risk of putting the students in more danger by trying to help without the required knowledge and skill. To support this, he has been working hand in hand with the Heart Safe Program, initiated by the local Fairfield medical centre. This program is aimed at training students and teachers on how to respond to cases of Sudden Cardiac Arrest. Students are trained on how to recognize when a sudden cardiac arrest attack is happening, how to administer CPR and finally how to contact EMS. With this kind of training, these attacks will claim less and less lives in the school environment. The advantage of this program goes beyond the fact that students get to know on how to respond to serious attack situations. It also creates the opportunity through which others get to know of a disease that is relatively not known especially to affect children. This sensitization of people in the society creates an awareness that will help curb the deaths from this disease in the school environment and in homes among other environments.

Conclusion

Sudden cardiac arrest has caused an increase in the death of teenagers especially in schools. It is important to note that effective responsiveness in an attack situation could save the lives of these children. Jerry Stebelton of the Ohio House of Representatives is doing a commendable job in trying to improve the environment to curb death due to sudden cardiac arrest. He has however not looked into the standardization of the pre-participation sports assessment but is inclined towards further research in the area leading to discussion on the subject. The government should conduct constant assessments in order to evaluate the implementation of the policies that are passed. This will ensure that all policies in one sector work together towards the set goals other than some policies not being implemented while new ones are developed. These alternative policies however does not undermine the need to continue campaigns on changing lifestyles in aspects such as what people eat and situations in which they work. These are still very serious contributors to the increase in cardiovascular diseases in the nation and the entire world.

The hospital clinical documentation improvement (CDI) specialist reports to you, the HIM manager of Anywhere hospital, that the hospital has been receiving reimbursement penalties. The physician documentation is not appropriately identifying specific conditions that CMS has identified as hospital-acquired conditions (HACs). In most cases, these conditions are present in patients before they are admitted. Anywhere hospital has a robust EHR. Many ­documentation improvement initiatives have been leveraged by the technological capabilities of the EHR. As the HIM manager, you wonder if something could be done from a technology standpoint that could assist physicians with identifying, upon admission, those conditions that are causing the reimbursement issue, and appropriately documenting the conditions. You are a HIM professional working in Anywhere hospital’s HIM department. You have been asked to review physician documentation within the hospital’s new EHR system, implemented six months ago. The goal of the review is to catch any documentation issues early and work with the appropriate hospital leadership to fix those issues.

As you review the documentation in the EHR, you notice that physicians are utilizing the copy and paste functionality available in the EHR, which allows them to select health record documentation from one source or section of the EHR and replicate it in another source or section of the EHR. In one instance the health record identifies a patient as a 65-year-old male (as identified during the registration process), but in the progress notes the patient is described as a 25-year-old female who has given birth. Clearly, the physician utilized the copy and paste functionality inappropriately and accidentally copied health record information from the health record of a 25-year-old female and pasted that information into the health record of a 65-year-old male.

This type of error could have patient safety concerns, as well as billing and claims issues, and the use of this functionality could open up the facility to potential claims of fraud and abuse by the payer. In general, this assignment requires students to (1) identify, examine, and analyze a current event that can be associated with mental health using psychological perspectives that have been discussed in class; and (2) outline at least one way to increase the public’s knowledge of mental health issues. 

For Part 1 of the paper, each student should: 

  • Select a current issue or event that is related to a mental health issue.  This event has to have occurred (or been in the media) within the last year.  Students can focus on a particular mental health issue or approach it more generally.  In other words, this current event may stand on its own or stem from social, political, religious, environmental, or other concerns.  
  • Choose two perspectives that have been discussed over the course of the semester (e.g., biological, cognitive, social, developmental, behavioral, psychodynamic, etc). Using these two perspectives, compare and contrast them regarding the selected current event.  In other words, students must analyze the pros and cons of each perspective as they apply to the issues.  This portion of the paper should:
    • Discuss how these perspectives explain what is occurring with regard to this mental health issue.  (How can the concepts/theories selected be used to explain the current event/issue?  How well do they explain the event?)  
    • Organize arguments to reveal differences and similarities in the views; include insights and information from relevant sources.
    • Use sources to support ideas and back up factual claims. Your psychology textbook is an appropriate source.
    • Be mainly analytical.  Although a portion of the paper will be devoted to describing the current event/issue, as well as to a general discussion of the psychological perspectives, the vast majority of the paper should consist of the student’s analysis.
  • Discuss which perspective best explains the current mental health issue, based on the student’s analysis.  
    • Each student must critically analyze and explain why the chosen perspective is a better choice than the other.

For Part 2 of the paper, each student should:

  • Describe a way to increase the public’s knowledge of mental health issues.  This strategy must be based on the perspective that was argued in Part 1 as the best explanation of the current event.  
    • Refer to relevant psychological research that supports this proposed strategy. Your psychology textbook is an acceptable source of research.
    • Discuss possible limitations of the proposed strategy.

Even if the government loses additional positions, the public uproar over the loss of service to tiny metro centres throughout America will likely draw their attention. Moreover, there has been a steady growth in the popularity of staycation in Canada for some time now. Toward the end of 2020, Ottawa promised millions of dollars to increase its visibility in the country (Berthiaume, 2020). At a time when Air Canada’s domestic market faces several financial and logistical challenges, this is welcome news. The future of this business, which generated more than $100 billion to the Canadian economy prior to COVID and supported 1.8 million employment (Press, 2020), is dependent on a coordinated strategy for rehabilitation and reopening that incorporates involvement from the tourist industry. Airfare in Canada is expensive, according to Lindeman (2019). Travelling from one part of the nation to the other might be pretty expensive. Also, it has been stated that Canadians spend more than double the amount that Americans do for domestic flights that are of comparable length. There is a lot of money to be made by Air Canada even if many customers find it difficult to travel  on their planes.
